Positions: Professional Aging Life Care Manager™ and Life Care Manager

Position Title:
Professional Aging Life Care Manager™ – Private Care Management and Guardianship Program

FLSA Status:  Non-Exempt
Travel:  Minimal
Location: Two positions, one in Austin, TX and one in Houston, Tx

Position Summary:
The Professional Aging Life Care Manager™ is responsible for providing comprehensive care management services to clients. Complete assessment, planning and care are provided in compliance with federal, state, and national guidelines/regulations and standards of care related to the Aging Life Care Association, Texas Estates Code and the Judicial Branch Certification Commission. Business services include both Private Care Management and Guardianship of Person for all clients. The Professional Aging Life Care Manager™ will be responsible for a caseload which may vary and include the elder client, those with developmental disabilities, mental health concerns, or individuals with chronic physical disease or special needs. Clients may reside in their homes or in a community setting. Care and resource management is provided with the goal of ensuring quality and safe delivery of care.

Position Qualifications:
1. Hold’s a Bachelor’s, Master’s or Ph.D. with at least one degree in a field related to Aging Life Care™/care management, i.e. counseling, mental health, gerontology, nursing occupational therapy, physical therapy, sociology, or social work. Licensure in Social work required for supervision of intern staff.
2. Required Minimum of two years post degree supervised experience in direct practice, administration, or supervision of client centered services in the field of Aging Life Care™/ care management
3. Highly organized, ability to work in non-traditional, highly professional work environment and manage fee-for-service caseload effectively
4. Outstanding interpersonal and communication skills
5. Knowledge and comfort using electronics including smart phone and laptop
6. Ability to drive, reliable access to transportation
7. Organizational and IT skills and experience with Microsoft Word, Excel and Outlook programs
8. Clear national and state criminal history record and clean motor vehicle driving record

Licenses and/or Certifications:
1. Texas Guardian Certification to be obtained within 2 years of hire; Provisional Certification to be obtained upon hire.
2. Meets requirements for Professional or Advanced Professional Member of Aging Life Care Association
3. Certified in or meets requirements for Certification in Care/Case Management.
4. Meets all State license requirements for professional status.

Position Responsibilities:
1. Conduct professional, objective assessments of older adult clients and their families, including psychosocial history; in-person meeting; communicating with community partners as needed
2. Develop professional, effective relationships with clients, families, responsible parties and care providers in all care settings
3. Create and effectively communicate client-centered professional, objective, creative recommendations for care, resources, education and support to clients and families
4. Initiate, establish, and maintain client files and electronic records.
5. Supervise interns as assigned by supervisor
6. Assists in implementation and oversight of recommendations
7. Assists with representation of company in Court in guardianship hearings providing testimony as required.
8. Provide client-centered problem-solving, ongoing support and advocacy for clients as needed both when client facing and non-client facing to continuously drive client care forward
9. Participates in weekly staff meetings/case review
10. Arrange for and/or accompany clients to medical appointments, provide feedback to family and ensure that medical orders are followed through as needed
11. Document billing and interventions daily
12. Comply with HIPAA confidentiality and privacy guidelines.
13. Attend professional development programs sufficient to obtain and maintain licensure and certification
14. Apply for and renew licensure and certification as required by relevant governing bodies
15. Abide by Standards of Practice for Aging Life Care Professionals as determined by ALCA
16. Follow other duties as assigned

Environmental Working Conditions:

Ability to work successfully in a fast-paced, occasionally stressful environment, utilizing good and sound judgement and patience in all situations. Work settings may include but not limited to office, client residence, courts, hospitals, and psychiatric or long-term care facilities. Position may require travel for client visits in service areas. Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration, and initiative. May be required to lift or transfer from ten (10) to fifty (50) pounds, carry, stoop, reach, bend, or kneel. May be exposed to mechanical and electrical equipment, chemicals, fumes, gases, odors, and some toxic waste. Able to perform basic job requirements safely and successfully with ADA, FMLA, and other state, federal and local standards.

Position: Thoughtful Engagement Specialist

The Thoughtful Engagement® Program is a personalized activity program for seniors designed to enhance quality of life through social enrichment, personal connection, and ample amounts of fun. We work with each client to create a customized engagement plan based on personal preferences, strengths, and limitations. These personalized plans include both in-home visits/activities, and special outings of our clients’ choosing. We provide the program’s services regardless of diagnosis, disease progression or activity level. We serve clients living in private homes, group homes, assisted living, and long-term care facilities.

Position Title:  Thoughtful Engagement Specialist
Reporting to:  TEP Director
FLSA Status:  Non-Exempt
Location:  Austin, TX
Travel:  Minimal

The Thoughtful Engagement Specialist is responsible for working directly with clients and providing services as part of the Thoughtful Engagement® Program.

Position Summary and Responsibilities

  • Works one-on-one with clients to provide life enrichment activities through the Thoughtful Engagement® Program.
  • Works with Thoughtful Engagement Coordinator to determine appropriate programming for each client.
  • Committed to providing exceptional customer service and adding value at every opportunity when working with clients, families, and other members of client’s care team.
  • Creative individual who enjoys learning about, researching, and participating in a wide array of topics, interests, and activities.
  • Ability to “keep the ball rolling” maintain a high level of energy in planning, developing, and executing individual activities for several clients on an on-going basis, without sacrificing attention to detail or level of service.
  • Ability to adapt to situations as needed (being flexible).
  • Ability to demonstrate/employ considerable patience and compassion in working with clients, serving as a key member of their support team.
  • Ability to build and maintain rapport with individuals whose life choices and views differ considerably from their ow
    Ability to accurately document client preferences, history, ongoing enrichment participation and concerns/challenges regarding all clients participating in the program.

Requirements

Education, Experience and Qualifications

  • Associate’s or Bachelor’s degree preferred, however 3+ years relevant experience may be substituted for degree
  • 2-3 years working with the senior population directly in creating and executing engagement activities is preferred.
  • Experience/education relating to healthcare, recreational therapy, life enrichment/activity planning, and social work strongly preferred. All relevant experience and education will be considered.
  • A deeply compassionate individual who enjoys working with seniors and is committed to bringing out the best in them.
  • An individual with an unyielding positive attitude that is able to impart motivation and a sense of fun, while building trust and rapport with clients and their families.
  • Excellent written and verbal communication and presentation skills, with the ability to effectively interact with all stakeholders, including senior leadership.
  • Excellent computer skills (especially Microsoft Office, with exceptionally strong concentration on Excel), excellent communication skills (both oral and written) and demonstrate strong client-orientation.
  • Must be proactive, detail-oriented, know how to solve problems and execute in a timely fashion.
  • Able to multi-task, prioritize and manage projects from concept to completion.
  • Must pass background check and pre-employment drug screen
  • Must provide proof of car insurance

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.

Mathematical Skills

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
